Pure speculation on my part. Fuel slosh cause fluid hammer and rupturing pipe joints or engine components.

Starship is keep fuel on board for return burns when engines shut down the vehicle stops accelerating, but the fuel keep moving and slams around the tank.

And with hot staging (starting the 2nd stage rocket motors before stage separation), there is much more stress on the first stage. We don't know much about the second stage.
