Here is my opinion of how the swiss cheese holes are lining up.

Amazon hires the lowest bidders for delivery, trucking and air freight. Nothing wrong with that but they skip important legal and safety matters.
Air 21 a new air freight charter service, likely created just for a Prime Air contract, early indication are it has safety issues.
The pilots the Captain less than 8000 hours total time, and less than a year in the 767 the co-pilot less than 3000 hours and about 18 months in the 767.
They were way behind the aircraft the entire approach, detail is above video. They should have by 767 procedure gone around at 1000 ft because of an unstable approach. Everything they did was too late.
